Position Summary
Flexible work arrangements, including virtual/hybrid work locations, are an integral part of the School of Medicine's Working Forward initiative. As such, this position's work location is designated as hybrid. Please note that the designated work location is subject to change based on the unit's business needs. If you would prefer workspace access outside of clinic hours, this position includes access to shared workstations at the CIDD.

Our goal is to help clinical care teams provide more efficient, effective, and family-centered care that optimizes quality of life and well-being for children and youth with Attention-Deficit/Hyperactivity Disorder ( ADHD) and their families. This position will focus on but not be limited to supporting Trillium connected families receiving care at UNC Pediatric outpatient services, CIDD clinics, and identified community practices (referred to as clinical teams going forward)., This position will also serve as a professional peer resource for families seen by primary care providers (PCPs) in outpatient services and clinics. In addition, this Trillium supported Family Navigator (FN) will be linked to the continually expanding network of UNC and NC based system family navigators via separate yet related projects.

This position will serve as a family system navigator. This position works directly with patient families to provide emotional, informational, and instrumental support to acquire desired services, resources, supports, and therapies. The FN duties and roles include and are not limited to the following: Supporting families (with emphasis on Eastern NC/Trillium connected families) directly to identify family and patient needs, work with relevant referring UNC pediatric medical providers, liaison and develop key partners and organizations in North Carolina, support the teaching of other UNC pediatricians and pediatric resident physicians, medical students, and LEND trainees about ADHD and common ADHD support needs, connect and/or network with similar efforts such as Duke Pediatrics program, and interface with other family system navigators from related projects at UNC, Duke, and elsewhere, assist in the ongoing development of FN training materials, assist in the tracking and monitoring and reporting of key outcomes. Additional responsibilities include: conduct needs assessment with patients and family to identify needed resources, communicate identified needs with clinical teams, build and provide tailored support for clinical teams, patients and families, initiate and respond to contact from families and community service providers, and help patients, families, and providers increase their knowledge of programs and systems as well as develop their advocacy skills. FNs may also receive and track referrals and document outcomes of support provided, demonstrate accurate, clear, professional, and timely communication, and engage in regularly scheduled team meetings, peer support, and supervision. FNs will document or communicate in Epic electronic health record, Microsoft Word, Microsoft Excel and Outlook email. FNs will adhere to all HIPAA compliance and patient confidentiality rules. FN's will engage in at least weekly clinical team meetings in which they will report back progress, share new information gathered from the family, and advocate on behalf of the family's self-determined and expressed priorities.
Minimum Education and Experience Requirements
Associate's degree in a related Human Services discipline and two years of experience related to the area of assignment, or equivalent combination of training and experience. All degrees must be received from appropriately accredited institutions.
Management Preferences

  • Have 'lived experience' as a primary caregiver or decision maker with experience in service system or education system navigation for a relative with ADHD
  • Ability to work independently with minimal supervision either in an office setting or from a remote location with regular team communication through Microsoft Teams and Outlook
  • Willing and able to do patient and family outreach calls regularly
  • Strong interpersonal and communication skills
  • Experience navigating North Carolina school, government (Mediciad, SSI, SSDI, EIPD) or healthcare systems into or beyond the transition to adulthood (child age 14+)

Special Physical/Mental Requirements
Physical ability to move from vehicle to clinic settings when needed. Physical ability to use, read, manipulate laptop computers. Duties to be performed with or without accommodation.
