OVERVIEW The Case Manager/Care Coordinator helps older adults and their families access community resources and long-term care services by conducting in-home assessments, developing individualized care plans, coordinating services, and determining program eligibility. This role serves as an advocate for participants, monitors changing needs, assists with Medicaid and other benefit programs, and helps ensure clients receive the support needed to remain safe and independent. Schedule: 40-hours per week ESSENTIAL DUTIES AND RESPONSIBILITIES
EDUCATION AND/OR EXPERIENCE Case Manager minimum qualifications shall be a minimum of 1 year experience in needs assessment and care planning, and: Bachelor's degree in social science, social work, nursing, or related field One year of program experience, may replace one year of college education up to and including four years of experience replacing a baccalaureate degree; or LPN with one year of program experience CERTIFICATION/LICENSURE BLS certification required within 90 days Successfully complete IL Department on Aging Care Coordinator Certification Maintain a valid driver's license and reliable transportation with proof of vehicle insurance when requested. Have and maintain an acceptable driving record per Mercy's HR-01 Fleet Safety policy. OTHER SKILLS AND ABILITIES Basic PC/Laptop skills Knowledge of Microsoft Office, Internet Explorer, Google Chrome Excellent customer service skills Effective verbal & written communication Organized, able to multi-task in a fast-paced environment Able to sit, stand and walk intermittently Ability to lift/carry laptop and paperwork to/from office and homes/facilities Must be able to navigate various entrances to homes/apartments/facilities PAY RANGE: $19.14 - $30.63Mercyhealth is an Equal Opportunity Employer.
