Position Details

The office is based in Sharon Hill and will service clients residing in Delaware County, PA. Caseload not to exceed 30 active clients.TheSubstance Use Disorder Case Manager, under the supervision of the Program Manager, is an individual who advocates for and connects individuals to community resources to provide opportunities and supports for individuals with mental health and co-occurring challenges. The Case Managerwill work with community resources such as DPW and SSA, mental health and drug and alcohol programs, adult and juvenile probation, payers, and the Office of Behavioral Health.

The ideal candidate will possess the following qualifications:

• A bachelor's degree in Social Work or a related field.
• 1 year direct care experience with the drug and alcohol / SUD population preferred.
• Clean, valid DL and reliable transportation.
