Lee Health is seeking a compassionate, relationship-focused Healthy Start Social Worker to support mothers, infants, and families throughout Southwest Florida. This hybrid role offers the opportunity to provide meaningful community-based services while enjoying the flexibility of a four-day workweek.

Strong preference will be given to candidates with prior experience conducting site visits, home visits, or community-based outreach with the populations served. Candidates who have experience building relationships with families in home or community settings are highly encouraged to apply.

Schedule

  • Monday Friday
  • Four 10-hour shifts (8:00 AM 6:00 PM)
  • One flexible day off each week
  • Hybrid schedule consisting of community home visits, remote work, and collaboration with the Healthy Start team at the Westlinks office

About Healthy Start

The Healthy Start Program is dedicated to improving outcomes for mothers, babies, and families through education, advocacy, care coordination, and community support.

This role is ideal for a social worker who believes in meeting families where they are, providing compassionate support, connecting them with vital resources, and helping them achieve healthier futures.

Position Summary

As a Healthy Start Social Worker, you will serve as a trusted advocate for families by providing case management, conducting community and home visits, coordinating services, and connecting clients with community resources.

Responsibilities include:

  • Conducting home visits and community outreach
  • Assessing family needs and identifying barriers to care
  • Developing individualized service plans
  • Connecting clients with community resources and support services
  • Providing ongoing case management and follow-up
  • Educating families on available programs and healthcare resources
  • Collaborating with healthcare providers and community partners
  • Participating in team meetings and program initiatives
  • Maintaining accurate and timely electronic documentation
The Ideal Candidate

Required Qualifications Education
  • Bachelor's Degree in Social Work (BSW) required

Candidates without a Bachelor's Degree in Social Work cannot be considered for this position.

Additional Requirements
  • Valid driver's license
  • Reliable transportation for community and home visits
  • Ability to effectively utilize electronic documentation systems
Preferred Qualifications
  • Previous experience conducting site visits, home visits, or community outreach with the populations served strongly preferred
  • Bilingual (English/Spanish)
  • Experience in maternal-child health, family services, case management, healthcare, public health, or community-based social services

Responsible for the delivery of services to the Healthy Start client population as outlined in the Healthy Start guidelines. The Healthy Start Social Worker facilitates services across a continuum through collaboration with community resources, the Healthy Start team, and other professional disciplines to ensure effective and efficient delivery of Healthy Start services and the achievement of desired client outcomes. Supports the organization vision and mission. Uses knowledge of patients age and cultural diversity into the delivery of services rendered. Contributes to the success of the Healthy Start team through excellence in customer service and performance improvement techniques that demonstrate positive outcomes of said services. Teamwork, flexibility and professionalism, while important in all functions of the System, are central to the success of the Healthy Start Team and are expected in this role.

Requirements

Education: Bachelors degree in social work required.

Experience: 1 year of social work experience in medical or healthcare setting preferred.

Other: Strong interpersonal/communication skills and organized work habits required. Must possess the ability to effectively negotiate computerized documentation.
