What You Need to Know

The Quality Clinical Data Specialist is responsible for assisting in planning, organizing, and implementing clinical quality management and data delivery. Ensures compliance with state, federal and accreditation standards. This position is a member of the hospice quality team and responsible for providing expertise in data gathering, data analysis and process evaluation. This position contributes to quality customer service and is responsible for relationship building, partnership with hospice site leadership.

If you meet these qualifications, we would love to meet you:

  • Graduate level degree in discipline
  • Minimum 2 years of Hospice or Home Health experience
  • Computer literate: working knowledge of Microsoft Office (Word, Excel, PowerPoint)

Responsibilities:

As a Quality Clinical Data Specialist - Hospice, you will:

  • Assist in developing standardized, pro-active survey, clinical audit, and review procedures, data collection tools and reporting and follow-up processes for standard survey and reviews.
  • Instructs and directs staff in developing and utilizing QI data, collections procedures, analysis, methods and systems.
  • Instructs agency personnel regarding compliance standards and best practices in the generation of appropriate, accurate and thorough documentation of patient care.
  • Develops necessary training programs and training materials to meet agency quality goals.
  • Determines best metrics for evaluating success (in preparation for audit and ongoing agency performance) and follow -up schedule.
  • Ensures timely and accurate reporting and responses to compliance-related issues and assists monitoring the implementation of corrective action plans related to such issues.
