Summary of Position

  • Community Health Worker bridges the gap between health care systems and their patients. They often work with vulnerable populations, ensuring that they have access to care and programs that can help them lead healthier, safer and more secure lives.
  • By performing their job duties well, Community Health Worker can help eliminate disparities in care to make healthcare systems more equitable. By helping with the coordination of care, they assist patients with connecting to community resources.
  • A Community Health Worker is a frontline public health worker who is a trusted member or has a particularly good understanding of the community served. CHWs serve as a liaison between health and social services and the community to facilitate access to services and to improve the quality and cultural competence of service delivery.

Principal Accountabilities

  • Conduct and manage Social Determents of Health (SDOH) screenings, ensuring all results are accurately completed and uploaded to the Unite US (referral) platform.
  • Conduct outreach and engage patients to identify health and social needs and identify patients in need of support, build rapport, and assess barriers to care through in-office screening, outreach, or community interactions.
  • Coordinate care and connect patients to appropriate community and clinical resources: Completed social needs screenings, referrals, and ensure patients can access services related to social needs (i.e., food insecurity, housing, and other supports.
  • Provide health education and promote self-management: Educate patients on available programs in ways that are culturally and linguistically appropriate to address their social needs.
  • Maintain accurate documentation and track patient progress: Record interactions, prepare reports, and update patient information in electronic systems to support care coordination and quality improvement efforts.
  • Collaborate with interdisciplinary care teams: Participate in team meetings, share patient updates, and work with clinicians, care teams, and population health staff to address patient needs.
  • Performs other job-related duties as required.

Qualifications

Education, Training, Licenses, Certifications

  • Possession of a high school diploma or equivalent; AND
  • Two (2) years of work experience in the health care, community health, or social services field; OR
  • Two (2) years of college-level training in health occupations

  • Interpersonal skills to promote positive interactions with patients and health care professionals
  • Strong communication, organization and collaboration skills
  • Interviewing skills to assess patients' needs
  • Organizational skills to maintain accurate records for multiple patients
  • Strong oral communication skills to convey health information in an easy-to-understand manner
  • Problem-solving ability to help overcome obstacles to care
  • Computer skills to use email, word processing, spreadsheet and patient records management software
  • Epic EMR experience
