Social Worker - Emergency Department (2:30 p.m. - 11:00 p.m.) Summary The Social Worker I provides direct psychosocial care, discharge planning, and care coordination for Emergency Department patients from 2:30 p.m. to 11:00 p.m., collaborating with patients, families, physicians, nurse case managers, and the interdisciplinary team. Services include psychosocial assessment, goal setting, behavioral interventions, crisis intervention, patient/family education, and referral to community resources to mitigate barriers to care and discharge. Compensation will be commensurate with equity and experience for roles of similar scope and responsibility.
