Home Care Registered Nurse, RN- PRN Day Shift 8am-4:30pm Flexible Schedule - Up to 8 hours per week - Occasional weekend/Holiday requirements

Ecumen Home Care-Lakeshore is hiring a Registered Nurse, RN. Ecumen Home Care - Duluth provides in-home and community-based services in the Minnesota Duluth area and is located in Duluth, Minnesota. The Registered Nurse position is responsible for the delivery of client care including triaging client needs over the phone and making visits to provide care to clients in their home.

Essential Job Responsibilities:

1. Makes visits to clients home to provide nursing care based on the client needs and their care plan. These visits could be scheduled or as needed.

2. Assesses the client's condition and reports any concerns to the clients Ecumen RN Case Manager. Collaborates with the RN Case Manager to update the care plan as needed and/or as directed by the medical provider.

3. Administers medications and treatments as prescribed by the medical provider, following the plan of care. Completes charting as appropriate for type of visit.

4. Communicates with appropriate medical providers regarding client needs and/or changes in the client's condition. Seeks orders from the medical provider as required.

5. Provides health care instructions to the client as appropriate per assessment and care plan.

6. Receives and responds to phone calls during assigned shifts.

Driving Requirements: Local Travel and Driver's License Required

Registered Nurse HC Minimum Required Qualifications:

* Graduate of an accredited Registered Nursing program

* 1 year of nursing experience in post-acute care setting

* Must be licensed as a Registered Nurse within the state in which working and must maintain licensure.

*Valid Drivers License, proof of insurance and satisfactory motor vehicle record

*Ability to communicate effectively both verbal and written

* Proficient with Microsoft Office Suite, OASIS Documentation

* Able to follow all safety rules, regulations, policies and procedures of the facility including but not limited to: Vulnerable Adult and Abuse Policy, HIPAA, Home Care, Bill of Rights, OSHA, Universal Precaution, AWAIR, Right to know, and SDS, ADA HUD policy and procedures, Fair Housing laws and regulations.

Preferred Qualifications:

* Bachelor of Science degree in Nursing

* One year nursing experience in Home Care

* Public Health Nurse certificate
