The compensation offered to the candidate selected for the position will depend on several factors, including the candidate's educational background, skills and professional experience.

Job Overview:

The Sr. Program Assistant provides direct social service support to patients as well as assesses and plans complex discharge planning needs for the Adult Psychiatric Inpatient Unit.

Qualifications:

Required:

  • Bachelor's degree in Behavioral Health or related field of study
  • At least 3 years of experience in behavioral assessment, planning and intervention and at least 1 year of experience with managed care authorization and review process

Scheduling Requirements:

  • Shift working, Monday-Friday 8:00am-4:00pm
  • Full-Time (FT)

Essential Functions:

  • Interfaces with social work team to collaborate on social supports, housing and treatment needs upon discharge
  • Participates in interdisciplinary treatment team planning rounds to assess discharge planning needs, identifies obstacles and communicates ongoing interventions
  • Educates and engages patients and families with availability and eligibility criteria for outpatient programs/services benefits to meet their unique needs
  • Creatively negotiates, coordinates, and advocates for patients with community-based service to assure the highest level of needed support, continuation of care and wrap-around services
  • Collaborates with the team to identify, send, and follow-up on all appropriate referrals
  • Builds and maintains relationships with outpatient referral agencies and programs that will best serve our patient population
  • Conducts weekly discharge planning groups
  • Documents all discharge planning related notes and referrals in the Medical Record
  • Other job duties as assigned
