Description

Health@Home is an innovative home-based primary care program developed in partnership with the Neighborhood Health Plan of Rhode Island and members' primary care teams. The program delivers evidence-based care that is both high-quality and cost-effective, aiming to enhance the quality of life for Rhode Island members.

The Community Health Coordinator provides administrative and coordination support to the community-based, home-based primary care team serving Neighborhood Health Plan members with complex medical and social needs. The care team is comprised of Nurse Practitioners and Community Health Workers.

This role is responsible for coordinating and facilitating key functions that support patient engagement and access to medical, behavioral, and social services. The Community Health Coordinator serves as a liaison for the community-based care team, effectively utilizing available resources to support care coordination and address members' social needs. Working collaboratively with clinical and community-based staff, this role helps ensure the delivery of high-quality, cost-effective, patient-centered care.



Duties and Responsibilities:

Responsibilities include, but are not limited to, the following:



  • Answer inbound phone calls from patients, caregivers, providers during practice office hours and take appropriate action. Documents in EMR and maintains communication community-based care teams.
  • Schedules, reschedules and manages health care team's appointments and completes daily reminder phone calls.
  • Completes outreach calls to potential patients via telephone and completes patient registration upon acceptance. Within outreach call process researches for updated patient demographic information.
  • Serves as main contact for community-based care teams.
  • Assesses and cross matches outlook calendars and EMR calendars for complete information of patient visits
  • Support assigned duties for administrative processes including but not limited to monthly departmental mailings that including outreach letters, primary care provider letters, discharge letters and returned mail
  • Use list/resources to track hospital admissions, ER visits, transitional care management & efficiently contacts patients to schedule visits based on patients' needs and practice protocols
  • Supports process for all authorizations & consents including, labeling documents in EMR and compiles with yearly HIPAA consent audits.
  • Support process for patient survey results for the Health@Home program
  • Maintains appropriate written and oral communication and documents patient encounters and updates the EMR within established time frames.
  • Maintains patient confidentiality within legal and regulatory parameters
  • Other duties as assigned

Required:



  • High school diploma or equivalent
  • Two (2) years' experience in health care setting (i.e. Community Health Center, Medical office, Hospital).
  • Computer Literacy: Strong PC and Microsoft Office, including experience in Outlook and Electronic Medical Record (EMR) experience
  • Ability to communicate and relate to individuals and families of various ethnic and cultural backgrounds, ages and economic status
  • Self-motivated with strong organizational, prioritization and time management skill
  • Intermediate to Advanced skills in Microsoft office suite; Word, Excel, PowerPoint, Outlook, internet
  • Strong interpersonal skills
  • Strong clerical skills, data entry skills and attention to detail, ability to keep accurate records and prepare reports
  • Ability to problem solve and exercise sound judgment
  • Ability to work independently and as part of a team
  • Ability to work independently or in team environment with collaborative and customer service approach
  • Ability to provide solution-based thinking while exceed productivity standards for the department
  • Knowledge of medical terminology
  • Strong oral and written communication skills including excellent customer service


Preferred:



  • Medical office background
  • Clinical experience
  • Experience with EMR
  • Bi-lingual in Spanish-fluency, (read, write and speak)
