Job Summary: |
This position is based in the Infectious Disease Group Practice at the University of Colorado Hospital. The Retention Coordinator provides retention in care and access adherence monitoring services to patients living with HIV. Key Responsibilities:
Assess patient needs for retention services by reviewing clinical information, conducting outreach, and identifying social, behavioral, and systemic barriers to care. Provide retention and re-engagement services, including outreach, follow-up, appointment coordination, and development of individualized retention plans to support continuity of care. Maintain regular communication with patients to promote adherence to care and treatment plans and track patient progress and outcomes. Document all assessments, services, and outcomes accurately in the electronic medical record and relevant tracking systems. Support care coordination by assisting patients with referrals to internal and external services, including mental health, substance use, housing, and community resources. Collaborate with clinical teams, support staff, health departments, and community partners to ensure continuity of care and facilitate patient re-entry into services. Participate in quality improvement, research, and program development activities, including data collection, monitoring program effectiveness, and identifying opportunities for service improvement. Ensure all activities comply with organizational policies, confidentiality requirements, and regulatory standards.

Hybrid - this role is eligible for a hybrid schedule of 3 days per week on campus and as needed for in-person meetings. |

Qualifications: |
Minimum Qualifications:
Applicants must meet minimum qualifications at the time of hire. Preferred Qualifications:
Bachelor's degree in public health, social services, or another related field. Professional experience providing services to people living HIV. One (1) year of case management or related experience.
Knowledge, Skills, and Abilities:
Ability to communicate effectively, both in writing and orally. Ability to establish and maintain effective working relationships with employees at all levels throughout the institution. Outstanding customer service skills. Strong organizational and multitasking skills. Ability to work independently as well as collaboratively within a multidisciplinary team with diverse populations. Strong work ethic with the ability to be flexible. Ability to adhere to professional standards of conduct and ethical practice.

Anticipated Pay Range: |
The starting salary range (or hiring range) for this position has been established as $56,555 - $71,939. The above salary range (or hiring range) represents the University's good faith and reasonable estimate of the range of possible compensation at the time of posting. This position may be eligible for overtime compensation, depending on the level. Your total compensation goes beyond the number on your paycheck.
