Description
The Complete Health Nurse is a key member of the Lifespark Medical Group team that serves Seniors in their place of home (Residential home, Independent Living, Assisted Living, Memory Care, Group Home). The Complete Health Nurse provides comprehensive care coordination for our members while delivering targeted nursing tasks that support care plans, quality outcomes, and utilization reduction. The Complete Health Nurses support the delivery of expert geriatric primary care by taking a whole-person approach to delivering home-based medical services. They will work collaboratively with a broader interdisciplinary team of providers and other allied health professionals to improve their member's health. Position: Complete Health Nurse Salary: RN Salary Wage Range: $87,000 - $90,000 annually, depending on experience LPN Hourly Wage Range: $26.00- 32.00 per hour, depending on experience Service Area: East and Northeast Metro. Plus, mileage reimbursement of 72.5 cents per mile. - East Metro will consist of: Woodbury, North St. Paul, Lake Elmo and surrounding. - Northeast Metro will consist of: Zimmerman, Arden Hills, Fridley, Maplewood and surrounding. Schedule: 1.0 FTE, Full - Time / Monday - Friday, 8:00am-5:00pm Lifespark Benefits Include:
