Description

The Complete Health Nurse is a key member of the Lifespark Medical Group team that serves Seniors in their place of home (Residential home, Independent Living, Assisted Living, Memory Care, Group Home). The Complete Health Nurse provides comprehensive care coordination for our members while delivering targeted nursing tasks that support care plans, quality outcomes, and utilization reduction. The Complete Health Nurses support the delivery of expert geriatric primary care by taking a whole-person approach to delivering home-based medical services. They will work collaboratively with a broader interdisciplinary team of providers and other allied health professionals to improve their member's health.

Position: Complete Health Nurse

Salary:

RN Salary Wage Range: $87,000 - $90,000 annually, depending on experience

LPN Hourly Wage Range: $26.00- 32.00 per hour, depending on experience

Service Area: East and Northeast Metro. Plus, mileage reimbursement of 72.5 cents per mile.

- East Metro will consist of: Woodbury, North St. Paul, Lake Elmo and surrounding.

- Northeast Metro will consist of: Zimmerman, Arden Hills, Fridley, Maplewood and surrounding.

Schedule: 1.0 FTE, Full - Time / Monday - Friday, 8:00am-5:00pm

Position Functions & Responsibilities:



  • Deliver excellent customer service to members, partners, and colleagues.
  • Manage a clinical panel with focus on high-risk members through individualized care plans and active collaboration.
  • Coordinate care across the continuum, including specialists, home care, hospice, SNF/TCU, senior living, and community resources.
  • Monitor members during high-risk transitions with post-discharge follow-up, medication reconciliation, and service coordination.
  • Support primary care providers through pre-visit preparation, triage of member concerns, escalation, and follow-through.
  • Partner with providers to close preventive and quality care gaps (AWVs, screenings, vaccines).
  • Provide clinical oversight including medication management, wound care, skilled nursing interventions, and chronic disease education.
  • Perform ordered skilled tasks (e.g., immunizations, injections, labs, INR checks, wound care, catheterization).
  • Document all care, coordination, and communications accurately and timely in the EMR.
  • Communicate directly with MDs/NPs/managers for urgent needs and respond promptly via phone, EMR, Teams, email, or fax.
  • Participate in team meetings and provide coverage for other clinical staff as needed.
  • Maintain strict confidentiality and compliance with PHI standards.


60-70% is Care Management & Coordination

30- 40% is Task based Nursing Care

Additional Functions



  • Understands and complies with all the rules and regulations that govern the responsibilities of a registered nurse/licensed practical nurse. Completes required oversight documentation and escalates assessment needs to medical team as needed
  • Completes orientation and annual training requirements assigned
  • Participates in Lifespark's Event Management process including initiating reports, escalation of events and investigations
  • Promotes safety in the workplace through real time coaching and participation in safety awareness training
  • Schedules and keeps member appointments
  • Organizes travel efficiently and submits accurate auto use logs by established deadlines
  • Tracks all billable activities



Qualifications:

Education



  • Registered Nurse or Licensed Practical Nurse
  • Prefer advanced certification as a Care Manager, Holistic Nursing, Geriatric CM, and/or Gerontology


Experience



  • 2 years' RN/LPN experience in community-based services (assisted living, home care, hospice, public health)
  • Experience working with seniors preferred
  • Experience in customer service/direct member contact field preferred
  • Experience in multiple settings and situations
  • Experience in long-term goal setting and proactive intervention
