DETAILED JOB DESCRIPTION * Directs the daily operations of the Ambulatory Care Management department, including staffing, development of policies and procedures, and implementation of professional communication practices. * Designs and oversees the implementation of care management workflows, documentation tools, and billing processes. Collaborates with Information Technology to ensure effective documentation systems and data reporting structures. * Guides the focus and work efforts of RN Nurse Navigators, Behavioral Health Consultants, and LPN Nurse Navigator Associates to support value-based care initiatives. * Monitors quality and financial outcomes of value-based programs. Identifies performance gaps and coordinates improvement strategies across departments. * Ensures compliance with regulatory and contractual requirements for care management activities. For Medicaid Medical Home (MMH) programs, maintains NCQA accreditation for eligible clinics and supports the onboarding of new sites. * Conducts routine data reviews and oversees quality reporting for Medicaid Medical Home (MMH) and Chronic Care Management (CCM) programs. Applies analytics to monitor performance and inform decision-making. * Establishes and maintains professional relationships with internal and external stakeholders, including post-acute care, payer organizations, and governmental bodies, to facilitate care transitions and program alignment. * Leads a culture of safety through proactive risk mitigation and continuous quality improvement, taking measures to evaluate regulatory readiness routinely. Monitors and analyzes safety and performance metrics to identify trends and implement corrective actions for staff and/or patients as appropriate. * Manages the development, coordination, and maintenance of daily staffing schedules to ensure the appropriate level of coverage and continuity of care. * Monitors and oversees time and attendance in alignment with MU Health Care policies and practices, ensuring accuracy of records and timely approval for payroll purposes. * May complete unit/department-specific duties as outlined in department documents. REQUIRED QUALIFICATIONS * Bachelor of Science in Nursing degree. * Missouri Board of Nursing RN or Nurse Licensure Compact multi-state RN. When primary state of residency changes, compact state RNs must apply under the new state of residency within thirty (30) days. * Five (5) years of clinical nursing experience. * Five (5) years of experience in one or more of the following areas: clinical integration, managed care, physician practice leadership, physician practice clinical experience, quality management, IT/data management, or client-facing healthcare consulting. PREFERRED QUALIFICATIONS * Master's degree in healthcare administration or a related area. * Additional license/certification requirements as determined by the hiring department. PHYSICAL DEMANDS The physical demands described here are representative of those that must be met with or without reasonable accommodation. The performance of these physical demands is an essential function of the job. This position is very active and requires ambulating, reaching, and moving objects weighing up to and over 20 lbs. This role also routinely comes into contact with patients who may have contagious illnesses. This position is in a health care facility open 24 hours a day, 365 days a year.
