Description
What You Will Do to Change Lives The Case Manager Substance Use Counselor Certified serves as the primary support and case manager for individuals with mental illness by assisting them to live as independently as possible in their chosen community, focusing on substance abuse and addictions. Hours of operation are 8:00am-7:00pm; Monday - Saturday Schedules Available: Monday- Friday, or work Saturday with the option to take one day off during the week. Shifts Available: Full Time; 10:30am - 7:00pm Expected starting wage range is $26.46- $32.69. Telecare applies geographic differentials to its pay ranges. The pay range assigned to this role will be based on the geographic location from which the role is performed. Starting pay is commensurate with relevant experience above the minimum requirements. Program Type: Supportive Housing To Make Referrals: All referrals to The Shores and Pacific Village are prioritized and matched by the region's Coordinated Entry System. # of Units: 75 Who We Serve: Adults experiencing chronic or literal homelessness and have a disabling condition substantially limiting one or more major life activities (as defined by HUD Category 1). Hours of Operation: Monday - Friday from 8:00 a.m. - 7:00 p.m. About the Program: In partnership with San Diego Behavioral Health Services and San Diego Housing Commission, Telecare offers onsite supportive and case management services for persons residing at The Shores and Pacific Village. Service participation is voluntary and not a requirement of residing at The Shores and Pacific Village. Telecare works with tenants to engage them in services according to their behavioral health and case management needs. Services are provided throughout the community based on preference: on-site in The Shores and Pacific Village PSH community space; in individual units; in the supportive service office; or in the community, if appropriate.
