Overview

The Adherence Advocate is a customer service professional that will be responsible to organize and collect information from patients in follow-up telephone contacts and to track and report on individual and aggregate clinical information. The Adherence Advocate will apply positive communication skills with internal and external customers. The Adherence Advocate will follow established pharmacy law (and nursing law if applicable to the associate) to include Federal, NYS, and other applicable states, and procedures for health promotion and education and be accountable for delivering cost effective high quality care.

Qualifications
  • High School diploma or GED required; Certified Pharmacy Technician or NYS licensed LPN preferred.

  • Three (3) years of pharmacy experience required.

  • Demonstrated ability to identify barriers to a successful care management path.

  • Demonstrated ability to interact effectively with physicians and other members of the health care system.

  • Proficient PC and Windows skills required, including MS Office.

  • Excellent problem-solving abilities and time management skills.

  • Excellent written, verbal and interpersonal communication skills.

  • Demonstrated transferable knowledge, skill and ability to complete job duties independently and proficiently. Flexibility in work schedules and assignments.

Essential Accountabilities
  • Facilitate a systematic approach to support surveillance and educational outreach.

  • Assemble and review clinical records obtaining information from multiple sources including MD offices, direct telephone call to patients and internal data systems.

  • Forward any patient that falls out of the acceptable, defined range to a Pharmacist for case review.

  • Utilize defined assessments assigned by a pharmacist to screen for potential issues. Report any issues to a pharmacist.

  • Initiate contact with patients and follow protocol to obtain accurate and timely information to facilitate positive outcomes and determine patient tolerance to therapy.

  • Documents and Reports adverse events, adverse effects, and lack of tolerability to MD, pharmacy and management team.

  • Use computer programs including CPR +, MS Office, (word, excel, power point) to record data and create reporting structure.

  • Assist in the maintenance of Prior Authorization forms.

  • Maintain positive facilitative communication and problem solve difficult situations to resolution.

Hiring Compensation Range: $22.50 - $26.00 hourly

Compensation may vary based on factors including but not limited to skills, education, location and experience.

In addition to base compensation, associates may be eligible for a scorecard incentive, full range of benefits and generous paid time off. The base salary range is subject to change and may be modified in the future.
