Compensation: We offer our employees a competitive compensation and benefits package that reflects our organizational culture, mission, and core values. The salary range for this position is $95,160.00 to $100,638.72 and may be commensurate with experience.

The Behavioral Health Consultant (BHC) is an embedded member of the primary care team who utilizes the Primary Care Behavioral Health (PCBH) model to deliver same-day, brief behavioral health assessments and interventions for patients referred by Medical, Behavioral Health Services (BHS), and other SOME programs.

The BHC facilitates warm handoffs, promotes whole-person, integrated care across Medical, Dental, and Substance Use Disorder (SUD) Treatment programs, and provides consultative support to clinicians to improve population health outcomes.

Required: Master's Degree in Social Work; LICSW or LPC; 3-5 years of experience providing services in addiction treatment and mental health; experience providing services to underprivileged populations.

Required License/Certification: LICSW or LPC by the District of Columbia Department of Health; CPR/First Aid certified

Schedule: Monday - Friday, 8:00 am - 4:30 pm (40 hours per week)

Expected Contributions:



  • Conduct assessments and brief interventions for behavioral health conditions in primary care in accordance with the Primary Care Behavioral Health (PCBH) Model
  • Accept same-day referrals from medical providers
  • Utilize evidence-based interventions to assist clients in creating behavior change plans
  • Follow up with patients when appropriate, or recommend follow-up with PCP
  • Train new medical providers on the use of the PCBH model
  • Coordinate and communicate with medical staff regarding patient care
  • Document, track, and report on BHC interventions, including pre- and post-measures of patient wellness whenever possible
  • Provide training and supervision to interns in the PCBH model as assigned; provide clinical supervision to social work interns and LGSWs if eligible
  • Maintain appropriate client contact hours as outlined for face-to-face interactions, billable interventions, and care coordination services.


Mental Health Treatment Services



  • Provide crisis intervention services for clients experiencing a crisis
  • Conduct intake and other clinically appropriate assessments as needed to understand clients' current functioning and support needs
  • Work in collaboration with other SOME staff to ensure collaboration and care coordination of shared clients
  • Work in collaboration with the psychiatric prescribing staff at SOME to coordinate care and ensure continuity of care for clients


Addiction Treatment Services



  • Provide brief interventions for medical patients experiencing substance abuse issues. Assess clients for Substance use as applicable. Refer to treatment as appropriate
  • Facilitate groups for addiction treatment programs as needed
  • Provide addiction counseling, as needed


Outreach



  • Attend monthly IBHC meetings to exchange information on integrating behavioral healthcare in primary care settings
  • Provide psycho-education and support to other SOME staff
  • Build case loads through internal SOME referrals and partnerships with other programs
  • Work with other SOME staff to provide support, intak,e and assessments for other SOME programs as necessary

  • Knowledge of mental health and substance abuse issues
  • Experience with or ability to learn brief interventions for behavioral issues in primary care
  • Ability to work in a fast-paced primary care environment

Reports to: SVP, Federally Qualified Health Centers

Position Designation: This position is designated as Safety Sensitive. You may be subject to drug testing before or during your employment with SOME. In this position, you may be disqualified from employment based on the presence of marijuana in test results, even if you possess a medical card authorizing the use of medical marijuana.

Physical Demands: Must be able to lift up to 20 pounds. May be required to sit or stand for long periods. Must be able to move around all levels/floors of the building.
