Summary The Social Worker provides direct psychosocial care, including assessments, care planning, counseling, crisis intervention, and discharge planning. Working collaboratively with patients, families, physicians, and the interdisciplinary team, the role addresses psychosocial barriers, engages patients/families in care, and connects them with resources to optimize outcomes. Essential Functions
Conducts screenings, psychosocial assessments, and develops care plans.
Provides counseling, crisis intervention, grief support, abuse/neglect reporting, and resource referrals.
Coordinates care and discharge planning with the multidisciplinary team.
Maintains timely and accurate documentation per regulatory standards.
Facilitates family meetings, interdisciplinary rounds, and community resource connections.
Mentors junior staff; may supervise students and social workers per state regulations.
Participates in quality improvement initiatives and provides coverage as needed.
For Community Psychiatry Program: independently diagnoses, completes intakes, monitors treatment effectiveness, and complies with Joint Commission standards.
Qualifications
Education: MSW required.
Licensure: Valid LCSW-C required.
Skills: Strong assessment, counseling, crisis intervention, care planning, and conflict resolution skills. Knowledge of community resources and the psychosocial impact of illness. Proficiency in electronic health records and excellent communication skills.
