Description
The MTM Clinical Pharmacist is responsible for providing patient-centered medication therapy management services with the goal of optimizing therapeutic outcomes, improving medication adherence, and minimizing adverse drug events. This role involves conducting comprehensive medication reviews (CMRs), targeted interventions, and collaborating with patients, caregivers, and healthcare providers. The pharmacist uses clinical expertise to identify medication-related problems and develop individualized care plans, ensuring safe, effective, and cost-efficient medication use.
Qualifications
Minimum Education Doctor of Pharmacy (Pharm.D.) Doctor of Pharmacy (Pharm.D.) degree from an accredited institution (Required)
Minimum Work Experience 3 years Relative clinical pharmacy experience (Required) And 1 year Successful completion of a PGY-1 residency accredited by the American Society of Health Systems Pharamacists (ASHP). (Required) Or 0 years Successful completion of an accredited medication therapy management certificate program (Required)
Required Skills/Knowledge Exhibit strong communication and interpersonal skill Detail-oriented and proficient in medication therapy management and patient counseling. Strong problem-solving skills and knowledge of drug interactions, side effects, and pharmacokinetics. Ability to identify areas of operational improvement and growth opportunities Ability to evaluate and make decisions on the appropriate therapy for patients, considering individual factors such as comorbidities, allergies, and other medications. Ability to engage patients in conversations that promote adherence to medication regimens and lifestyle changes. Ability to work effectively with physicians, nurses, and other healthcare professionals to optimize medication therapy. Skill in navigating disagreements or differing opinions within the healthcare team regarding treatment options or patient care. Ability to identify drug-related problems such as drug interactions, side effects, contraindications, and therapeutic failures and provide effective solutions. Ability to assess complex clinical situations, analyze relevant data, and make informed decisions regarding medication therapy management. Skill in accurately documenting interventions, patient interactions, and changes in therapy to ensure compliance and continuity of care. Ability to conduct comprehensive medication reviews in a timely manner while ensuring high standards of care. Familiarity with using electronic medical records to access patient histories, document interventions, and communicate with the healthcare team. Ability to use pharmacy-specific software tools to track medications, monitor adherence, and manage patient information. Knowledge of MTM practices, including the comprehensive medication review (CMR), targeted medication reviews (TMR), and interventions to address medication-related problems. Understanding of various chronic disease states (e.g., diabetes, heart disease, COPD, hypertension) and their pharmacological treatments, including their monitoring and management strategies Ability to identify high-risk medications and implement strategies to mitigate risks (e.g., drug monitoring, patient education).
Required Licenses and Certifications Pharmacist District of Columbia 90 Days (Required) And Pharmacist Maryland Upon Hire (Required) And Pharmacist Virginia 90 Days (Required) Cardio-Pulmonary Resuscitation (CPR) 30 Days (Required) .
Functional Accountabilities
- Provide Medication Therapy Management and/or disease state management to a specific panel of patients in accordance with pharmacy board and state regulations
- Critically evaluate a patient's medication profile to ensure that each medication is indicated, effective, safe, and that the patient is adherent to therapy
- Completing timely documentation of activities within the electronic medical record as appropriate for specific areas of responsibility
- Maintaining accurate and proper patient medication profiles, monitoring to ensure the proper choice of medication, dosage form, strength, length of therapy, and interaction potential of the prescribed medication
- Provide medication information to patients, nurses, physicians, or other health care practitioners
- Critically evaluate and interpret medical information when providing specific information to physicians or providers
- Precept Resident or PharmD student rotations
- Maintain current knowledge of pharmacotherapy, pathophysiology, and procedures in the area of practice
- During patient encounters, responsible for the interpretation of provider medication orders for accuracy and appropriateness, informing them of any errors, omissions, or discrepancies
- During patient encounters, based on an assessment of the patient's specific medical needs, making interventions with the medical staff and nursing staff to facilitate optimal drug therapy and facilitate the provision of other pharmacy services
- May administer vaccinations, as applicable
- May work under Collaborative Practice and protocols as authorized by providers
- Adhere to specific organizational and regulatory guidelines regarding the provision of MTM services, ensuring compliance with all legal and reimbursement requirements.
- Contribute to the management of pharmacy budgets by promoting the use of cost-effective medications and avoiding unnecessary treatments or tests.
