What You Will Do - Essential Functions
As a Provider at Emcara Health, you'll deliver comprehensive care to patients in their homes and/or community-based services such as assisted living or skilled nursing facilities, helping to improve outcomes and promote continuity of care.
Please note the position requires you to meet patients in their homes throughout Orange or Lake County, FL (depending on where you reside) and you will be expected to cover a territory radius of 50 minutes from your home address.
- Clinical Care & Patient Management:
- Conduct in-home visits (minimum 25/week), including assessments, exams, and medication reviews.
- Manage chronic conditions and develop individualized treatment plans.
- Serve as the primary care provider and conduct follow-ups.
- Maintain accurate EMR documentation and comply with healthcare regulations.
- Patient Engagement & Collaborative Care
- Educate patients on disease management and healthy living.
- Collaborate with interdisciplinary teams to ensure coordinated care.
Who You Are
Required Qualifications:
- Active, unrestricted licensure as a Provider in the state in which you will be providing care for patients. License must be clear of any clinical and/or board practice restrictions.
- Certification as a Family Nurse Practitioner, Adult-Gerontology Nurse Practitioner or Physician Assistant.
- 1+ year of experience in primary care or a related field such as family practice, general practice, or outpatient care.
- DEA license and BLS certification
- Strong clinical and diagnostic skills.
- Proficiency in electronic medical record (EMR) systems.
- Reliable transportation and clean driving record.
- Ability to pass background check upon hire and throughout employment to include criminal felony & misdemeanor search, SSN validation/trace search (LEIE), education report (highest degree obtained), civil upper and lower search, 7-year employment report, federal criminal search, statewide criminal search, widescreen plus national criminal search, health care sanctions-state med (SAM), national sex offender registry, prohibited parties (OFAC) (terrorist watchlist), and a 10-Panel Drug Screen.

Competencies
- Must have strong computer skills, and both knowledge and proficiency with electronic medical records.
- Have a strong understanding of risk adjustment models for the Center for Medicare and Medicaid Services (CMS) and the Affordable Care Act (ACA).
- Demonstrate empathy and respect for members while completing in-home assessments
Working conditions:
- Ability to lift and carry up to 20 lb
- Mental and physical stamina to perform assessments and documentation.
- Work is performed in member homes and may involve exposure to varied conditions.
