RN, Population Health Nurse (Nurse Coach) - The Population Health Nurse, also known as the "Nurse Coach," plays a pivotal role in managing a broad patient population and driving performance across key quality metrics aligned with value-based agreements for ProHealth Care and ProHealth Solutions (PHC, ACO). This role is essential to achieving top-tier Star ratings and advancing system-wide quality outcomes. The Nurse Coach must bring a blend of clinical expertise and administrative skill, with strong proficiency in data managementnincluding advanced Excel and pivot table functionality.
