Job Description
General Purpose of Job: Evaluate patients, plan treatment programs, and provide physical therapy interventions to patients. Delegate andsupervise physical therapy activities performed by others. Essential Duties: This job description is intended to cover the minimum essential duties assigned on a regular basis. Team members may be asked to perform additional duties as assigned by their leader. Leadership has the right to alter or modify the duties of the position.
- Evaluate and assess patients based upon the referral / establish reasonable goals with patient and/or caregivers.
- Provide patient and/or caregiver education about the results of the evaluation, method of treatment and the results of treatment. Instruct patients and/or family members in home programs.
- Perform treatment according to the established plan, utilizing all available modalities, equipment and techniques within the scope of physical therapy.
- Performs medical record documentation according to established criteria.
- Re-evaluate patients, assess their progress, and make recommendations as to their discharge planning.
- Communicate with other health care professionals verbally and in writing.
- Delegate to Physical Therapy Assistants (PTA's), Athletic Trainers (ATC's), Rehab Aides, and students/interns.
- Supervision including directing, reviewing work performed, training, and cosigning notes for interventions provided by PTA's, ATC's, Rehab Aides, Volunteers, and students/interns ensuring that department policies and procedures are upheld and that quality patient care is provided.
- Cooperate with support staff and other health system departments in order to maximize the efficiency of non-clinical services such as the registering, scheduling, and billing of our patients.
- Participate in in services, staff meetings, team meetings, continuing education courses, and other meetings as needed.
- Follow Sparrow Hospital and Department specific policies, procedures, and compliance guidelines to ensure patient safety including, but not limited to: Infection Prevention, Privacy, and environmental safety.
- Supports and serves as a role model reflecting the Mission, Vision, and Values of the Hospital system
- Participates in On-call rotation coverage of Sparrow Ionia Hospital inpatient care for weekend/ holiday coverage and intermittent weekday coverage as needed.
- Provide Cross-Coverage to other Community Hospitals as needed.
- Performs other duties as assigned.
Job Requirements
| General Requirements |
Current Michigan license as a Physical Therapist Current BLS Certification required within 60 days of hire |
| Work Experience |
* Experience in an Outpatient Physical Therapy Setting, preferred. * Experience with a mix of Pediatric and Adult populations, preferred. |
| Education |
Graduate of an accredited Physical Therapy program. |
| Specialized Knowledge and Skills |
* Interpersonal skills to effectively and appropriately communicate with patients, families, physicians, nurses and other healthcare associates. * Demonstrate excellent verbal, written and oral communication skills. * Willingness to be trained in specialty areas as indicated by needs of the clinic to meet patient needs. * Manual PT skills, preferred. |
