Location: Dodson Medical Office BuildingDepartment: Cln-Hematology/OncologyShift: First Shift (United States of America)Standard Weekly Hours: 40Summary: Referral Coordinator is a registered nurse (RN) responsible for promoting the services and enhancing the reputation of Cook Children's Hematology and Oncology Department with a concentrated focus on relationship building and follow up with referral providers regarding current status of H/O patients. Referral Coordinator will use a collaborative process to plan, assess, implement, monitor and evaluate the options and services required to meet the healthcare needs of the patients using utilization review and case management skills. Referral Coordinator shall deliver competent and skilled care to patients and families according to their identified needs. Nursing care is based on the nursing process and augments the medical plan or treatment. The Referral Coordinator will deliver care with a team-orientation, and emphasis on good customer relations, sound clinical judgement and appropriate decision-making abilities that take into consideration evidenced based practice. Monday - Friday 8am - 5pm Duties:
