Position Title: HCBS Incident Coordinator

Work Location: Remote, Ohio

Assignment Duration: 6 months (Contract only)

Work Arrangement: Remote

Position Summary:

* Development, implementation and maintenance of initial and ongoing education as it pertains to Waiver Incident Reporting from start to finish.

* Identification and processing of reported waiver incidents as applicable and in alignment with state/federal regulations (Abuse, neglect, misappropriation, exploitation, health and welfare, among others)

* Oversight of Waiver Incident Management with state contracted investigation organization.

* Coordination of waiver incidents that do not fall to the stated contracted investigation organization.

* Inclusive of prevention plan evaluation and resolution.

* Audit Waiver Incidents to assure compliance and alignment for annual state/federal audit.

* Performs other duties as assigned.

* Complies with all policies and standards.

Qualification & Experience:

Education/Certification




  • Required: Degree and or certificate/licensure required in a field of Nursing, Social Work and/or Counseling. Two years of experience required. Must have knowledge of waiver and incident reporting.



  • Preferred:




Licensure




  • Required: Clinical licensure/certification is preferred: LPN, SWA, CHW, LSW or LPC. Clinical experience in lieu of licensure will be considered.







    Candidate Requirements

    Employee is expected to review and submit incidents and ensure that these are followed through and closed within the time period given in MyCare's 3 way contract CMS and Ohio Department of Medicaid and Medicaid's contract with the state of Ohio
