Responsibilities:

The Medical Social Workeris responsible for assessing developing, and implementing appropriate social work care plans which take into account the psychosocial, financial, community resource, long term planning and bereavement needs of hospice patients and their families. Functions as a key member of the interdiscipl nary healthcare team.

Qualifications:

• Masters degree in Social Work required.
• Current license from the State of New Mexico Licensed Master Social Worker-LMSW.
• 1 year hospital social work experience, protective services, or practicum in acute care settings experience desired.
• National Case Management certification or SW Palliative Care Certification; ACHP-SW or APHSW-C preferred.
• BLS required at time of hire.
